Led by specially trained professionals, our Brain Health and Well-Being Programs take a holistic approach, supporting each resident’s physical, emotional and spiritual needs. Through individualized care and enriching activities, we help residents stay active, engaged and connected. For those in the early stages of memory change, the cognitive Wellness Center provides additional support for both residents and loved ones.
Individuals experience memory loss differently — which is why Wesley Willows offers flexible programs designed to meet each person’s unique needs. From full-time residency to day programs and prevention for early cognitive support, our continuum of Memory Care options ensures compassionate, personalized attention at every stage.
Throughout the continuum of memory care, every activity is designed to support brain health, foster self-esteem and create meaningful moments of joy. Our caring team takes a holistic approach to engagement — encouraging participation from residents, families and friends alike. Each month’s calendar blends structured and spontaneous activities that nurture personhood well-being across each of the seven dimensions of well-being; physical, emotional, financial, social, occupational, spiritual, intellectual and environmental. Activities include:

Nurture emotional well-being through music and pet therapy, support groups and uplifting social interactions.

Enjoy culinary team-prepared meals and snacks designed to support brain health and follow Alzheimer’s Association guidelines.

Our higher levels of care provide a warm, neighborhood-like secure environment for individuals with Alzheimer’s disease and other forms of dementia. Residents and day program participants feel at home while receiving continuous, personalized care from our specially trained team.
Every detail of the community is intentionally designed to promote comfort and independence — with wayfinding features, gentle environmental cues and spaces that encourage ease and confidence.

The admission process for Memory Care at Wesley Willows begins with a thoughtful conversation, either by phone or in person, with a knowledgeable member of our team. This initial discussion allows us to learn about your loved one’s needs, answer your questions, and help determine the most appropriate next steps. Families are encouraged to schedule a tour at any point during the process to experience our environment and meet our caregivers firsthand.
Admission includes a comprehensive review of medical history, cognitive and functional abilities, personal preferences and financial considerations. This clinical assessment helps ensure that we can provide the right level of care, structure and support for your loved one’s safety and well-being. Our team will guide you through each step with clarity and compassion, coordinating timelines and supporting you throughout the transition to make the process as smooth and reassuring as possible.
Wesley Willows’ residents who wish to participate in our Memory Wellness Center must be independent in all self-care tasks and needs. Residents must also be independently mobile and able to complete transfers on their own. Finally, they must be able to administer their own medication since no nursing services are provided at the Memory Wellness Center.
Rockford is one of the best cities for dementia care in the country. In December 2023, the city was officially recognized as the 28th Dementia Friendly Community in the nation, as part of the Dementia Friendly America movement. A Dementia Friendly Community makes a concerted effort to bring increased awareness, education, resources, experiences and strategies for inclusion to the public. Wesley Willows is just one of several sectors of the Rockford community striving to make the region a more supportive and inclusive environment for people living with dementia and their loved ones.
One example of a special Memory Care program offered at Wesley Willows is our use of Montessori‑inspired programming, which focuses on preserving dignity, purpose and independence for each resident. This approach emphasizes engaging residents in meaningful, familiar activities that are adapted to their abilities, allowing them to experience success, choice and a sense of contribution throughout the day.
Wesley Willows offers a Virtual Dementia Tour for staff and family members. This tour provides a glimpse of the day-to-day life of living with dementia and an empathetic lens for staff to better understand what person-centered care entails.
We also have a monthly Coffee and Caregivers support group where experiences, both good and bad, can be shared in a safe space among friends and neighbors.
Wesley Willows, families and loved ones are always welcome to visit and participate in activities with their resident. We understand, however, that there is no single “right” answer for how often visits should occur in Memory Care, as each resident’s needs, history and comfort level are unique.
Maintaining familiar routines can be comforting, especially if your loved one is accustomed to regular visits. At the same time, as memory loss progresses, some individuals may experience increased fatigue, confusion or anxiety with frequent visits, and a more flexible approach may better support their emotional well-being.
