Willows Health & Rehab Center

Willows Health & Rehab Center is a Medicare-certified, comprehensive skilled care center with 24-hour staff including registered and licensed practical nurses; certified nursing assistants; physical, speech, and occupational therapists; registered dietitian; activities staff; and other specially-trained professionals.

Our Medical Director monitors all of the medical care provided to our residents. In-house appointments with physicians, dentists, ophthalmologists, and other specialists are available. In Willows Health & Rehab Center, as in all areas of Wesley Willows, each resident is provided an individualized, comprehensive program of care to encourage the highest level of independence and to promote personal dignity and quality of life.

In-patient and Outpatient Rehabilitation Therapy

In-patient and outpatient therapies are available at Willows Health & Rehab Center. Residents are encouraged to attain their highest level of independence through physical, occupational, and speech therapy. Often, individuals will stay short-term to recover from injuries or illness, and then return to their homes. Outpatient therapy services are available for those living at Wesley Willows or in the Rockford community.

Respite Care

For caregivers who need a break or have to be away for business or a vacation, we offer respite care—short-term care for your loved one. Professional care and services are tailored to your individual needs and include three delicious meals per day and a wide array of activities. Respite care is also a “mini-vacation” for your loved one, where they can enjoy time and interaction with others.

Hospice and Comfort Care

Wesley Willows has special rooms designated for those needing end-of-life comfort and services. We provide care for residents and family members so they can spend quality time together. Staff is trained to be supportive and sensitive to physical, emotional, spiritual, and social needs.

Our Comfort Care Room is available for short-term stays for family members as a respite when facing difficult decisions or transition choices. Wesley Willows is the only community in Rockford that provides this service where families can be together.
